FoodMaxx Opinion Survey

Recent customers to FoodMaxx should take part in their customer survey in order to earn a 10% discount code on their next visit.  To start the survey the FoodMaxx customer will need the following:

  1. Working Internet connection
  2. Survey invitation code (found on the FoodMaxx customer receipt)
  3. A valid email address

The feedback provided by customers will only be used to help increase the quality of customer service as FoodMaxx and WILL NOT be sold to any third parties.  The 10% discount promotion is limited to one per customer during the promotional period and cannot be used on tobacco, liquor, dairy, lottery, stamps, gift cards, phone cards, any type of ticket, donations and vending machine purchases.  The 10% discount survey must be used on the same purchases and cannot be divided up into more than one visit.
